Use open-ended questions sparingly
Open-ended questions take longer to answer, lower the number of families who finish, and are harder to summarize. They are most useful for explaining the rating questions. Use one to three, and place them after the rating questions. Two that work well together: “What is one thing we do well?” and “What is one thing we could improve?”
