• Online: Easiest to share and tabulate, and most free survey tools work well. It can miss families with limited internet access or little comfort with online forms. Most families will open it on a phone, so test it on one.
• Paper: Reaches families who won’t complete an online form. Handwriting can identify a family, and responses must be entered by hand. Provide a sealed drop box that staff do not handle.

Offering both formats reaches the most families. Keep the questions identical in both.
